Оценка риска для младенца сформирована исходя из различных источников, иногда противоречивых.

Минимальный

Препарат совместим с грудным вскармливанием. Высокий уровень безопасности для младенца или лактации.

Благодаря очень высокой молекулярной массе и высокой связывающей способности с белками плазмы препарат выделяется в грудное молоко в неопределяемом или клинически незначимом количестве.
Уровни препарата в плазме детей, чьи матери принимали инфликсимаб, были неопределяемыми или крайне низкими.
Не наблюдалось ни краткосрочных, ни долгосрочных неблагоприятных воздействий на развитие, иммунологические ответы и/или частоту инфицирования младенцев, матери которых принимали инфликсимаб.
Из-за отсутствия пероральной биодоступности попадание препарата в плазму младенца через проглоченное грудное молоко маловероятно, за исключением недоношенных детей и во время раннего неонатального периода, во время которого может наблюдаться повышенная проницаемость кишечника.
Многочисленные эксперты и научные общества ревматологии, гастроэнтерологии и дерматологии считают инфликсимаб препаратом очень низкого риска, совместимым с грудным вскармливанием.

Как определяется доказанная эффективность лекарственного препарата?

По принципам доказательной медицины препарат считается эффективным только при наличии положительных результатов клинических исследований высокого уровня доказательности. Эффективность препарата, обладающего даже огромным количеством исследований более низкого уровня не может считаться доказанной.

Как проводятся клинические исследования лекарственных препаратов высокого уровня доказательности?

В рандомизированных контролируемых испытаниях (РКИ) участников случайным образом распределяют по группам. Одни пациенты попадают в опытную группу, а другие в контрольную. За обеими группами ведут наблюдение в течение определенного времени и проводят анализ исходов, формулируемых в начале исследования. Эффективность лечения оценивается в сравнении с контрольной группой.

Анализ объединенных результатов нескольких РКИ называют мета-анализом. За счет увеличения размера выборки при мета-анализе обеспечивается большая статистическая мощность, а, следовательно, точность оценки эффекта анализируемого вмешательства.

Что такое терапевтическая эффективность лекарственных препаратов?

Стоит разделять понятия доказанная эффективность и терапевтическая эффективность лекарственных препаратов. Терапевтическая эффективность - это способность производить эффект (например, понижение артериального давления), доказанная эффективность достоверно подтверждает терапевтическую эффективность. Таким образом, в теории препарат может обладать терапевтической эффективностью не обладая доказанной эффективностью.

Какие критерии оценки доказанной эффективности препарата в проекте MedIQ?

В качестве критериев оценки наличия доказанной эффективности мы используем крупнейшие в мире и самые надежные базы данных препаратов и клинических исследований. В качестве примера можно привести «List of Essential Medicines» Всемирной Организации Здравоохранения. Если у препарата нет исследований, но мне он помогает, является ли он доказанно эффективным?
